Frequently asked questions

  • Do I really need a hip replacement? Are there other options?
    Alternatives to hip replacement are; painkillers, physiotherapy and steroid injection into the hip joint. The worse the arthritis is, the less effective the alternatives will be.
  • What type of hip replacement is best for me?
    The main decision is between cemented, hybrid (half-cemented) and uncemented. By far the most common in my practice is hybrid (cemented stem, uncemented socket). The decision is made based on your bone quality. The harder your bone, the more likely that uncemented will work for you. The bearing type is based on your age. For those aged under 70 years, a ceramic ball is preferable. Over 70yrs a metal on plastic articulation is likely to outlast you. 98% of modern hip replacements are still going after 22 years.
  • What are the risks and potential complications?
    Knee Infection 1%, stiffness/swelling (common), blood clots, fracture, major nerve or vessel injury less than 1/1000, dissatisfaction 10%, inability to kneel -common. Hip Infection 1%, dislocation 1%, blood loss needing transfusion 1/50, blood clots, implant failure, fracture, nerve injury -sciatic nerve 1/1000, leg length discrepancy, scar pain. General perioperative risks will depend on your pre-existing medical conditions.
  • How do I choose the right surgeon or hospital?
    Your surgeon should undertake a good volume of joint replacements. You should expect a minimum of 50 cases per year, with a minimum of 10 per year for partial knee. For added peace of mind, you can search for your surgeon’s numbers up on the NJR website. However, the surgeon’s outcomes are not published unless he/she wishes to do so. You can look up the hospital outcomes on the NJR (National Joint Replacement) website.
  • What kind of anaesthesia will be used?
    In over over 90% of cases, spinal anaesthesia is used.
  • What should I do to prepare for surgery?
    Pre-surgical physical therapy ("prehab"), staying fit - use an exercise bike, if available. Attend “Joint School” Prior to surgery, you will be asked to stop HRT or hormonal medications for 30 days to reduce the risk of blood clots. Blood thinners will also need to be stopped but these vary depending on the type of blood thinner and the indication for why you are taking it.
  • How long does the surgery take?
    Hip replacement surgery takes from 40 mins to 1 hour depending on whether the hip replacement is uncemented, hybrid (half cemented) or fully cemented. Bone cement takes approximately 10 minutes to set! A total knee replacement takes 45-50 minutes. A partial knee replacement takes 30-45 minutes.
  • What is minimally invasive surgery, and am I a candidate?
    Partial knee replacement is done through a minimally invasive approach. The incision for a hip replacement is as big as it needs to be to undertake the operation safely. A slim patient will have an incision that is approximately 10 cm long. In more obese patients, the incision may be closer to 20 cm.
  • How long is the recovery process?
    Most hip replacement patients go home the day after surgery. Hip patients usually use crutches for 2-3 weeks. Most total knee replacement patients go home the day after surgery. Knee replacement patients usually use crutches for 3-4 weeks. Many partial knee replacement patients can go home the same day (day-case). Partial knee patients are usually off crutches at 2 weeks. You can resume driving 30 days after joint replacement. Most people return to work by 30 days (desk job) or 90 days (physical job).
  • Will I need physiotherapy? For how long?
    Everyone has some physiotherapy after joint replacement. The amount varies from patient to patient. The more de-conditioned you are before surgery, the more physiotherapy you will need afterwards.
  • Will I need help at home after surgery?
    Many patients can cope even if they live alone. Crutches, assists and devices (grabbers/long shoe horns/toilet-seat raises) are provided. Arranging your home so you can live on the lower level (without stairs) may help for the first week. Likewise, a family member staying with you for the first week is helpful but not necessary.
  • What can I do to speed up my recovery?
    Prehab. Staying physically fit before surgery. Attending “Joint-School”, provided by the hospital before your surgery. Do the exercises!
  • How long will the hip replacement last?
    A modern design hip with a ceramic head should last 20-25 years Modern Knee replacements should last 15-20 years
  • Are there any activity restrictions after recovery?
    A hip or knee replacement is a mechanical device and like any mechanical device, is subject to wear and tear. If the hip or knee replacement is overloaded (obesity) it will wear out faster. High impact activity (road-running) will also wear out a joint replacement faster. In terms of keeping active, unlimited walking/hiking, unlimited cycling, cross-trainer, treadmill running, doubles tennis are all well tolerated.
  • What does a successful outcome look like?
    Pain relief, improved mobility, and return to doing the activities you want to do.
